The digital blockchain-based therapy management system for the medical practice. CE certified as a class I medical device in accordance with MDR.

How an App makes a successful system more user friendly, more efficient and more reliable and manages to annul the entire adherence problem.

Ruma GmbH specializes in adherence support in the field of addiction therapy.

The Ruma Marker-System replaces degrading direct observation of urine sample collection with marker substances. They allow the reliable matching of sample and sample donor.

Our extensive experience in this field serves as the ideal prerequisite to match the analog process to its digital counterpart.

We have transferred this analog process chain into the digital world by representing the individual marker substance as a digital twin in a private blockchain.

Now the digital twin documents the path of the substance from production to distribution to the physician, to the patient taking the substance, to validation of the urine sample by the lab, and finally to billing.

Application of the Ruma® Digital-System

From manufacturing to application: Ruma Digital-System ensures the whole therapy process.

1. Manufacturing
The Ruma® marker capsules are manufacrured individually coded with interlocking security features on outer and inner packaging and are distributed through laboratories to the associated medical offices/facilities. The codes are encoded again before being provided to the labs.

2. Dispensing
When introducing a new patient, the medical office/facility links the app of the patient to the app of the practice, thus individually activating the system for each patient. Data communicaton is executed in encoded fashion within a block chain. Dispensing of each individually coded marker is documented in a manipulation-proof manner. Through a push notification the physician defines the time frame during which the urine sample has to be provided.

3. Coding of the urine sample
The patient scans the QR-code* of their Ruma® marker and thus activates the personalized features of the patient's app. Going forward, they can only continue the process by scanning the code they have been assigned by their medical practice. The manipulation-proof intake of the coded Ruma® marker capsules guarantees the definite matching of urine sample and urine donor. The video of the intake is recorded, encoded and transfered to the treating physician/provider. The patient collects the urine sample independent of location and hands it over/sends it to the lab or practice.
*alternatively Data Matrix code possible.

4. Validation (physician)
The physician checks the marker intake video in an asynchronous manner for possible manipulation or correct intake, respectively, allowing them to promptly react if the patient/user needs support. Events suggesting manipulation are documented.

5. Validation (laboratory)
The lab performs the accredited analysis of the urine. This includes the confirmation of the marker code transmitted by the manufacturer as well as dedicated manipulation tests. The lab transfers the results to the physician.

The Consultation that doesn‘t require an Appointment.

The physician can check the digital recording of the patient's marker intake at his or her convenience. No need for trips to the medical office or waiting times. But instead more freedom for optimized office procedures.

After the intake process has been recorded the sample collector is notified accordingly.

He or she can now watch and check the video. The video is assessed by means of a defined scale that differentiates between „Successful Intake“ "Failed Attempt" or „Manipulation Attempt“.

Why Blockchain in a Medical App?

We use blockchain technology by EPN to ensure secure data transfer. It is the carrier of all information:

  • regarding the samples and their life cycle
  • regarding the sample donors
  • regarding sample collectors and their respective facilities

Plus, the history of the data is stored in their "life cycle" including all changes, administrative activities and protocols.

Every change can be traced back to a specific user so that these data are unforgeable and unchangeable. Data storage on the blockchain is audit-proof.
